腹腔镜手术治疗胆囊结石伴肝硬化的临床效果研究

摘要:
目的:探讨新鲜冰冻血浆、红细胞悬液不同配比对严重创伤出血患者预后的影响情况。方法:选取本院于2016年1月~2018年6月期间收治的严重创伤出血患者64例,随机分为研究组和对照组各32例。两组均提供常规急救措施,在此基础上研究组行等比例输血(即新鲜冰冻血浆:红细胞悬液=1:1),对照组严重创伤合并出血患者予以非等比例输血(即红细胞悬液:新鲜冰冻血浆=1.55:1)。比较两组24h、30d内死亡率、并发症发生率。结果:研究组24h死亡率、30d死亡率均低于对照组(P<0.05);研究组患者感染、ARDS、TRALI并发症发生率均显著低于对照组(P<0.05)。结论:应用红细胞悬液、新鲜冰冻血浆等比例输注急救严重创伤出血患者可有效降低死亡率、保障其获得理想预后。

Abstract:
To investigate the effect of different ratios of fresh frozen plasma and red blood cell suspension on prognosis of patients with severe trauma bleeding. Method:From January 2016 to June 2018, 64 patients with severe trauma bleeding were randomly divided into two groups: the study group (n = 32) and the control group (n = 32).Two groups of patients with severe trauma and bleeding were provided with routine first aid measures. On this basis, the study group performed equal proportion blood transfusion (fresh frozen plasma: red blood cell suspension =1:1), and the control group was treated with non equal proportion transfusion (red blood cell suspension: fresh frozen plasma =1.55:1).The mortality rate and complication rate in 24 hours and 30 days were compared between the two groups.Result:The 24h mortality and 30d mortality in the study group were significantly lower than those in the control group (P<0.05), and the incidence of infection and ARDS,TRALI complications in the study group were significantly lower than those in the control group(P<0.05).Conclusion:Application of red blood cell suspension and fresh frozen plasma infusion can effectively reduce mortality and ensure ideal prognosis for patients with severe trauma bleeding.
